This Participant Waiver Form shall extend to the following activities (“the Activities”):

1. Participating in or observing grain entrapment demonstrations and training, and all activities incidental thereto, including but not limited to my direct participation, being a spectator when not actively involved, and assisting others in any capacity related to carrying out training or attempted rescue activities;
2. Using equipment provided by the Canadian Agricultural Safety Association (“CASA”);

1. I understand grain entrapment is a dangerous matter that can result in personal injury and death, which is what training attempts to prevent. Training events involve simulated situations to enhance preparedness of emergency personnel to respond to entrapments, engulfment’s, entanglements, and asphyxiations in and around grain storage and handling facilities and these training events can present hazards with potential for injury and illness which include but are not limited to:
   • slips, trips, falls
   • cuts, burns and abrasions
   • exposure to high temperatures, exposure to dust and grain dust, and toxic dust including molds, exposure to infection agents, and exposure to flying debris
   • entrapment
2. Injury could be caused by a wide variety of different factors, including but not limited to direct participation in the Activities; the conduct of other participants, or spectators, or other factors. Injury may be the result of normal participation in the Activities, equipment failure, or the negligence or actions of people.
3. No matter what the cause, I voluntarily assume all risks associated with engaging in the Activities. I will not, under any circumstances, make a claim against CASA, its directors, volunteers, employees or agents (“the Released Parties”) for damage or injury that occurs. In the event that I do take any action this waiver shall constitute a full and complete defense to any claim against the Released Parties.
4. I hereby agree to hold harmless and indemnify the Released Parties from any and all liability for any loss, damage, expense, injury or cost that may result in any way from the Activities.

1. I declare that I am in reasonably good health, and am physically able to participate in the Activities. I declare that I will not participate in any Activities while under the influence of alcohol or drugs, or while in any condition that would diminish my capacity, and I take responsibility for any allergies I may have.
2. I acknowledge that this Agreement and Release shall be binding upon my estate, heirs, executors and assigns; and shall be governed by the laws of Manitoba, and any matters arising from it shall be adjudicated by the courts in Manitoba.
3. I hereby grant permission for my image to be used in promotional materials of CASA.
4. I am over 18 years of age.
